To become a member of CAST-UT, simply follow these simple steps:

• Download the CAST-UT membership application form (click HERE)
• Use Adobe Acrobat reader to open the file and print it
• Fill the form and mail it with the annual membership fee payable to CASTP to the following address:

• You will receive a confirmation by email or phone in two weeks
• You will receive your membership card shortly after
